Every hospice nurse who has worked through the HOPE Update Visits knows the rules. Day 0 is election. HUV1 lands on days 6 through 15. HUV2 lands on days 16 through 30. Score a symptom moderate or severe, and a follow-up visit is due within two calendar days. None of that is the hard part.

The hard part is what happens between people. The scheduler books a routine RN visit within the HUV1 window, but no one marks it as HUV1. A nurse scores pain at a 2, treats it well, and never realizes the clock on the follow-up visit has already started. A follow-up visit happens, but the original HUV record does not capture it correctly. Or the one that costs agencies the most: the nurse finds a real change, the doctor writes a new order, the HOPE record goes to Medicare on time and is accepted, and the written plan of care still reads exactly as it did three weeks ago.

Every one of those is a handoff failure, not a knowledge failure. Your nurses can be right about HOPE, and the workflow can still fail around them.

So what actually closes that gap? Not another training on how to score symptoms. A system that will not let a visit count as finished until the finding reaches the plan of care, and shows you exactly where the process is stuck when it doesn't.

The 30-day submission deadline is the one that affects your Annual Payment Update, and for an HUV it counts from the date the record was completed, including any follow-up visit, not from the visit date itself. The tracker calculates it correctly every time.

Separate HUV1 and HUV2 Documentation Prompts to ask for a current finding, a prior finding, and the date for each, so HUV2 compares against HUV1 rather than a stale admission picture. A HOPE Handoff and Action Agenda that carries findings into the IDG meeting with a named discipline attached to every open item, so nothing gets left as "the team will handle it." And an RN Competency Packet with knowledge questions, six scenario-based tests, an observed-visit checklist, a documentation review, and a remediation plan, for hire and for the annual re-check.
