Your nurses are good at their jobs. A surveyor cannot always tell that from the chart.

Here is the gap most hospice agencies do not see coming, even with strong clinical staff: care planning under 42 CFR 418.56 has been one of the most frequently cited hospice deficiencies since 2018.
