Your volunteers have to provide at least 5% of your total patient care hours. Not roughly 5%. Not "we have a great volunteer program, so we're probably fine." Five percent, calculated against paid staff hours, documented, and ready to hand to a surveyor. It's one of the few Conditions of Participation you can fail purely on arithmetic. Your volunteers can be trained, screened, supervised, and genuinely valued by your patients, and you can still walk away with a standard-level deficiency because nobody added up the numbers.

Here's the complication. That requirement was waived during the public health emergency. It came back on January 1, 2024.
